It starts with a caring conversation.
A patient, family member or primary caregiver can call us at any time, with or without a referral from a doctor. We have an experienced team standing by to listen to your needs and answer any questions you have regarding hospice care. Contacting us does not obligate you to go forward with hospice care in any way.
We consult with the primary physician.
With permission, we contact the patient’s physician to discuss the possibility and timing of hospice care. Our medical team works with the physician to develop a plan of care that addresses the patient’s individual needs and ensures that the experience is tailored to you and your loved ones.
You can focus on what matters.
The Serenity team makes this transition as easy as possible — so you can focus on what really matters. Hospice care can generally begin the same day we receive an order from the patient’s physician. In some situations this can be within hours of your call.
